Get scheduled
Becoming a patient starts with a call or an online request. New and returning patients are welcome, and walk-ins are seen the same day or at the next available time.
- Call (317) 291-7422 — phone hours are Monday–Thursday 8:00 AM–7:00 PM and Friday 8:00 AM–1:00 PM.
- Need to be seen today? Same-day (open-access) visits are available — call right at 8:00 AM.
- Walk-ins are always welcome.
Please note: if you arrive more than 15 minutes late, we may need to reschedule you for the next available time.
What to bring
Bring these to your first visit so we can register you quickly. Don’t have everything? Come anyway — we’ll help.
- Photo ID (state ID, passport, or visa) and proof of your current address dated within 30 days
- Your insurance card (Medicaid/HIP, Medicare, or private) and any co-pay your plan requires
- A list of your current medications and dosages, and any allergies
- Contact information for your other providers, plus immunization records
- Proof of guardianship or custody, if you’re bringing a child in your care

What to expect at your first visit
Please arrive about 30 minutes early to complete your new-patient registration.
- Your provider gets to know you — your medical and mental-health history and your life situation.
- You may be offered age-appropriate screenings (behavioral health, HIV, tobacco, and recommended cancer screenings).
- You’re matched with a primary care provider you’ll see going forward, for continuity of care.
- You’ll get set up on the patient portal to message your care team, view results, and refill medications.
How fees & affordability work
Shalom accepts most Medicaid and Medicare plans and private insurance. If you’re uninsured or underinsured, our sliding-fee discount sets your cost based on your income and household size. No one is turned away for inability to pay.
- Co-pays, if your plan requires one, are due at the time of your visit.
- The sliding-fee discount is open to all patients based on Federal Poverty Levels — bring your income and residency documents within 10 days to apply.
After your visit
Shalom becomes your medical home. You’ll see the same provider whenever possible, and your care team helps you set and reach health goals over time.
- Use the patient portal to message your provider, request refills, view results, and book follow-ups.
- Need to see a specialist? Your provider arranges referrals.
- We’ll encourage the preventive screenings and check-ups that keep you well.
