How your information is used
How we may use and share your information
We use and share your health information to provide your care and to run our health center, and in most cases we share only what is needed. Common reasons include:
- Treatment. To provide, coordinate, or manage your medical care and related services, including sharing information with other providers involved in your care.
- Payment. To bill and collect payment for the services you receive.
- Health care operations. For quality improvement, credentialing, training, compliance, and other operations that support your care.
- Appointment reminders & health services. To remind you of appointments and to tell you about treatment options or health-related benefits and services.
- People involved in your care. With family members or others who help with your care or payment, unless you ask us not to.
- Research. For research that has been approved through the appropriate review and privacy-protection processes.
- When required or permitted by law. In certain situations we may share information without your authorization — for example, for public health activities, reporting abuse or neglect, health oversight, legal proceedings, law enforcement, organ and tissue donation, coroners and funeral directors, workers’ compensation, military and national security, inmates, and to avert a serious threat to health or safety.
Extra protection for substance use disorder records. If you receive substance use disorder treatment at Shalom, those records receive stricter confidentiality protection than standard HIPAA records under federal law (42 CFR Part 2), and we generally need your written consent before sharing them.
When we need your permission
Uses that require your written authorization
Some uses and disclosures of your information are made only with your written authorization, including:
- Most uses and disclosures of psychotherapy notes.
- Uses and disclosures for marketing that involve payment to us from a third party.
- Any sale of your health information.
- Any other use or disclosure not described in this Notice.
If you give us written authorization, you may revoke it in writing at any time. Revoking your authorization stops any future uses or disclosures for the purpose you allowed.
Your rights
Your rights over your health information
You have the right to:
- Inspect and copy your medical and billing records, including an electronic copy of records we keep electronically.
- Request an amendment to correct information you believe is incorrect or incomplete.
- Request an accounting of certain disclosures we have made of your information.
- Request restrictions on how we use or share your information for treatment, payment, or operations.
- Request confidential communication — that we contact you in a specific way or at a specific place.
- Be notified of a breach that may affect the privacy of your information.
- Receive a paper copy of the full Notice at any time, even if you agreed to receive it electronically.
